Adverse drug reactions (ADRs) are unintended, undesired, and unavoidable noxious effecs of agents administered to patients for diagnostic, prophylactic, or therapeutic indications. Adverse drug reactions that are reportable are those that are considered serious enough to require a change in therapy (i.e., drug discontinuation, dosage modification, or additional therapeutic intervention). In accordance with WAC 246-873-080, in addition to placing the appropriate documentation in the patient's chart, whenever a reportable adverse drug reaction is suspected, the UW Medicine nurse, pharmacist, physician or other care provider should report the event via Patient Safety Net (PSN). All reports are promptly reviewed by the ADR pharmacist. Trends are noted and patient safety initiatives are developed as necessary to maximize safe medication use.
