  • HMC Adult Medicine Clinic Pharmacists: Two full-time pharmacists are available in AMC to provide clinical services Monday through Friday 8:00 to 5:00. An additional pharmacist is scheduled on a part-time basis to assist with all pharmacy functions. AMC pharmacists are committed to improving patient healthcare by providing safe and effective pharmaceutical care. Pharmacy services and activities include: providing general drug information/education to AMC providers and staff, patient education, direct provider consultation and refill authorization. Additionally, all patient medication profiles are reviewed by a pharmacist before each provider visit in an effort to proactively have positive impact on patients drug therapy. The adult medicine clinic is available to pharmacy residents and students as a clinical training site with AMC pharmacists serving as preceptors. The AMC clinical pharmacists are also available to assist in chronic disease state management and medication adjustment after provider referral. Pharmacists are involved in medication management, laboratory data monitoring, and continued patient education regarding drug therapy and disease states. The AMC pharmacists see approximately 250 patient visits each month. Pharmacist managed problems have included: thromboembolic disease and anticoagulation monitoring, diabetes, smoking cessation, hypertension, hyperlipidemia, asthma, and pain control.
