The Seattle Cancer Care Alliance'ss (SCCA) inpatient adult Hematopoietic Stem Cell Transplantation (HSCT) unit is located at UWMC and is a national and international referral center. This service cares for patients undergoing both allogeneic and autologous HSCT from their conditioning regimen through approximately the time of engraftment, patients receiving pre-transplant chemotherapy, patients who require hospitalization for pre-transplant disease-related complications, and patients who are readmitted for management of transplant complications. Clinical pharmacists participate in daily work rounds, monitor pharmacotherapy regimens, verify accuracy and appropriateness of chemotherapy orders, and provide education to patients and other healthcare providers. Pharmacists actively participate in supportive care issues such as pain management, antiemetic management, nutrition support, fluid and electrolyte management, and depression and anxiety.
