At UWMC, clinical pharmacy specialists provide services to the inpatient hematology/oncology unit. A wide variety of cancer types are represented by this geographically diverse patient population. Responsibilities to this service include monitoring and optimization of pharmacotherapy, pharmacotherapy education, and facilitating adherence to established treatment guidelines. Services include therapeutic drug dosing, parenteral therapy monitoring, home parenteral therapy coordination, medication compliance counseling, and discharge prescription teaching. At HMC, clinical pharmacy specialists provide services to the inpatient hematology/oncology unit as well as the outpatient oncology clinic located on the HMC campus. Inpatient responsibilities are similar to those provided at UWMC. Additional responsibilities include providing information regarding appropriate drug utilization and dosing regimens in the outpatient clinic setting. At the Seattle Cancer Care Alliance (SCCA), clinical pharmacy specialists provide services to the outpatient adult oncology clinic. SCCA is an ambulatory clinic that is a partnership of the University of Washington Medical Center, Fred Hutchison Cancer Research Center, and Childrens Hospital and Regional Medical Center. Pharmacy has a strong presence in the clinic, with patient care provided by clinical oncology pharmacists and a satellite pharmacy within the clinic.
