Clinical pharmacists provide anticoagulation therapy management for ambulatory patients at UWMC via the UWMC Anticoagulation Clinic. Anticoagulation management for other patients is provided by the pharmacist assigned to the patient's clinical service. The goals of management include minimization of thromboembolic disease recurrence and minimization of hemorrhagic complications.
