At UWMC, clinical pharmacy specialists provide services to both the general cardiology service and the heart failure/transplant service. Responsibilities to these services include participation on daily work-rounds, monitoring and optimization of pharmacotherapy, providing education to physicians, nurses and other healthcare professionals and collaboration in University research endeavors. Clinical services include development and monitoring of treatment protocols and clinical pathways, therapeutic drug monitoring, anticoagulation management and parenteral nutrition support. Direct patient-related services include self-administered medication (SAM) teaching for all heart transplant patients, medication compliance counseling, and discharge prescription teaching.
At HMC, pharmacists provide clinical services for patients in the Cardiac Intensive Care Unit on a consultant basis. In addition, this pharmacist responds to all cardiac arrest codes while on site. Both sites (UWMC and HMC) serve as rotation sites for fourth-year Pharm.D. students and pharmacy residents.
