Clinical pharmacists serve as integral parts of the Medical ICU teams at both UWMC and HMC. They are active in helping manage a very diverse range of patient populations at both medical centers. Responsibilities include daily work-rounds with the medical team, monitoring and optimization of pharmacotherapy, and provision of drug information and educational services to physicians, nursing staff, and other health care professionals. Pharmacists also participate in a variety of research initiatives in collaboration with UW Medicine researchers.

Clinical services include therapeutic drug monitoring for all patients, development and monitoring of treatment protocols and clinical pathways, anticoagulation management, and parenteral nutrition support. In addition, UWMC pharmacists help monitor post-op lung, liver, kidney, and pancreas transplant patients and provide self-administered medication (SAM) teaching for lung transplant patients. HMC pharmacists on the Medical ICU service respond to and play a major role in managing all codes in the medical center.

Both UWMC and HMC Medical ICU sites are rotation sites for many fourth-year Pharm.D. students and Pharmacy residents.
