The joint HMC/UWMC Refill Authorization Center is operated by the Departments of Pharmacy and Ambulatory Services to assist certain clinic patients in obtaining refills of chronic medications. It is staffed by clinical pharmacists. Using protocols, the pharmacist reviews the patient's chart, verifies medication dosage and strength, monitors appropriate parameters, and checks that the patient is compliant with return appointment schedules. Refill activities are documented in the patient’s chart and a copy of the transaction is forwarded to the patient’s physician for review. At HMC refill authorization is partially centralized. Using protocols, clinic-based pharmacists assist patients in obtaining medication refills and in several clinics.
