Comprehensive clinical pharmacy services are provided throughout the two hospitals and in selected clinic areas by clinical pharmacist specialists or by clinical pharmacy liaisons. Responsibilities include monitoring and optimization of pharmacotherapy, pharmacotherapy education, and facilitating adherence to established treatment guidelines. Services include therapeutic drug dosing, parenteral therapy monitoring, home parenteral therapy coordination, medication compliance counseling, and discharge prescription teaching. In addition, a UW School of Pharmacy faculty member is affiliated with the HMC Women's and Children & Teens Clinics.
- HMC Women's Clinic and Children & Teens Clinic Pharmacists: In this area pharmacists focus is on primary care issues, including providing care to patients with hypertension, diabetes, and asthma. Pharmacists provide patient counseling regarding HRT and birth control issues and closely monitor children with asthma, including making home visits when needed.
